Please, in order to fulfill your request within the scope of the Personal Data Protection Law (“KVK Law”), you must fill in the following application form clearly and completely and submit it to our Foundation in writing.

You can personally send a signed copy of the form to the address of Yüreklere Umut Vakfı (YUVA) Çınarlı Mahallesi 1583/2 Sokak No:2 B/Blok Kat:1 D:130 Konak/IZMIR, with the documents identifying your identity, through a notary public or in other areas specified in the KVK Law. You can send the form to yuva@yureklereumut.org with secure electronic signature.

We will respond to your application as soon as possible and within 30 days at the latest, free of charge. However, in case of additional costs, the Foundation reserves the right to charge you a fee according to the tariff to be determined by the Personal Data Protection Board. If the information and documents you have submitted to us are incomplete or incomprehensible, we will contact you to clarify your application.

This application form has been prepared to determine your relationship with our Foundation and to fully identify your personal data processed by our Foundation, if any, so that your application can be answered accurately and in a legal time. Our Foundation reserves the right to request additional documents and information (copy of identity card or driver's license, etc.) for identification and authorization determination, in order to eliminate legal risks that may arise from illegal and unfair data sharing and especially to ensure the security of your personal data. In the event that the information regarding your requests you submit within the scope of the form is not correct and up-to-date or an unauthorized application is made, our Foundation does not accept any liability for such false information or requests arising from unauthorized applications. All responsibility arising from unlawful, misleading or false applications lies with you.
